All requests for tax information must be made in writing, please complete the attached form.  Guests may turn in their completed Request for Tax Information Forms to the Rock Star Club for processing.  If the request is turned in Monday through Friday between 8:30am and 4:30pm, then the request will be processed within 20 minutes.  If the request is turned in outside of the listed timeframe the Compliance department can mail your request or leave it at the Rock Star Club to pick-up.

Please note:  Request for Tax Information Forms may also be mailed or faxed to the Compliance department for processing.
